A RESOLUTION TO TRANSFER TO THE PREFERRED (PPO) HEALTH CARE

PLAN UNDER CAREFIRST BLUECROSS - BLUE SHIELD

Introduced by: TOWN COUNCIL

WHEREAS, the current employee health plan administered by CareFirst BlueCross-BlueShield does not meet the healthcare needs of employees;

WHEREAS, the current employee health plan administered by CareFirst BlueCross-BlueShield is too expensive for the coverage provided;

WHEREAS, the current employee health plan by CareFirst BlueCross-BlueShield does not offer dental, vision, or life and disability coverage; and

NOW THEREFORE, BE IT RESOLVED that the Mayor and Town Council of the Town of Forest Heights, Maryland that the Town of Forest Heights will cancel the current health care plan administered by CareFirst BlueCross-BlueShield in order to enroll in a Preferred PPO healthcare plan that provides for dental, vision, prescription, life and short-term disability coverage through the CareFirst BlueCross-BlueShield, effective May 1, 2005.

AND BE IT FURTHER RESOLVED that this Resolution shall take effect upon its passage.
